As your needs change, you may find that your home needs to have adjustments made to help you remain independent for longer
Whether it’s due to a disability, long-term illness or getting older, you may be able to receive equipment or adaptations to help.
Minor adaptation
We may be able to adapt properties to meet your needs if the property is suitable for adaptation.
To support you to live safely and independently at home, we can provide home assessments completed by our Trusted Assessor for minor aids and adaptations.
They will explore your needs and consider what equipment you may benefit from. They will also provide information on other services and can make referrals. This is jointly funded through our Health and Housing Partnership with Livewell Southwest.
To request a Trusted Assessor home assessment, contact us on 0808 230 6500 or email us.
For very minor adaptations, where a home assessment is not needed, you can access our on-demand service without an assessment from an Occupational Therapist.
Minor adaptations include the installation of handrails, grab rails, bath seats and shower boards. A full list can be found in the On Demand Full Product List.
You can contact our supplier NRS directly on 0800 9887344 to place an order. They will deliver and install the items.
In-depth adaptation
Residents who need these more in-depth property adaptations must contact Plymouth City Council (PCC) or your local authority who work with partner organisations to help you continue to live independently in your own home.
PCC will work with Livewell Southwest who will carry out an assessment with an Occupational Therapist.
The assessment and most of the more minor alterations are free however you may need to contribute to the cost of some major adaptations, dependent on your household income. If you need help with funding these adaptations you may be eligible for a Disabled Facilities Grant. PCC and Livewell Southwest will assist you with this process.
To find out if you could benefit from any adaptations to your home, you can contact Plymouth City Council Adult Service Care on 01752 668000.

Moving to a more suitable home
Adapting your current home might not always be the best solution. There may be other homes already adapted or purpose built for people with disabilities that would suit your needs better.
If you do decide that moving is the right choice for you, get in touch with your Housing Officer who will discuss all the options available to you.
